Mark Dunn is a Pathology Quality & Service Improvement Manager at Harrogate and his role is to ensure regulatory compliance. Mark also looks after the implementation and management of the Pathology Department’s Quality Management System, including ensuring his organisation learns from any incidents.

When Mark attended the Silver Human Factors workshop the Harrogate team were thinking about using an in-house adapted version of the Yorkshire Contributory Factors Framework (YCFF) to investigate incidents. Attending the event reinforced the message that looking at the Human Factors contributory factors in incidents as they occur is very important for organisational learning.

Mark’s team plans to use the adapted Human Factors tool for the next few months and then to report back to the Trust Human Factors Group in summer 2019 with their findings. Mark intends to ensure his team has access to internal Human Factors induction, possibly through the Bronze online Human Factors training.

Mark attended the Improvement Academy Silver Human Factors training day, funded through the Yorkshire and Humber AHSN on 13th November 2018.
