Organisations engaged with eFI

The map below lists the GP practices, CCGs, other NHS organisations and industry partners engaged with the Healthy Ageing Collaborative. The map shows each engagement site and provides a brief description of how the site is using, or is considering using, the eFI to develop/test new models of  care for people with frailty in primary care.
